Follo Health was founded by Dr. Nimish Patel, a physician with more than 14 years of experience in hospital medicine.
Throughout his career, Dr. Patel has cared for patients during some of their most vulnerable moments. He has witnessed firsthand the dedication of physicians, nurses, and hospital teams working to stabilize patients and prepare them for discharge.
But he also recognized a recurring challenge.
Once patients leave the hospital or skilled nursing facility, the coordinated care they received can become fragmented. Medication changes, delayed follow-up appointments, unanswered medical questions, and communication gaps can create unnecessary challenges during recovery.
Despite the best efforts of healthcare teams, many patients are left navigating these transitions with limited support.
Dr. Patel believed there was a better way.
That belief became Follo Health.
Founded on clinical experience and a commitment to improving continuity of care, Follo Health was created to help bridge the transition between facility-based treatment and recovery at home.

His experience caring for patients in hospital and post-acute settings inspired him to develop a more coordinated approach to transitional care—one that supports timely physician follow-up, medication management, and continuity beyond discharge.
Through Follo Health, Dr. Patel is focused on combining clinical expertise, thoughtful technology, and efficient care coordination to improve how patients transition from healthcare facilities back into their communities.
